The legislation governing the transitioning of patients from hospitals to long-term care/community in Ontario has expanded significantly in recent years. This Guidance document aims to be a resource and promote a consistent approach and understanding of the information necessary to support and manage the transitions to care ensuring compliance with the legislative and regulatory requirements while recognizing both the patient's rights and responsibilities. The document details:
- The legislative framework for discharge planning
- The different roles and responsibilities of those involved
- Information to effectively managing the transitions in care
The document was jointly sponsored by the Ontario Hospital Association and the OACCAC and was written by legal counsel who regularly assists health care providers with issues related to patient transitions.
