Each year, Ontario's 14 CCACs file their Reports to the Community. They offer a snapshot—sometimes in multimedia form—of the patients and clients our members served, as well as their unique needs.
The reports also highlight the vital work that care coordinators, nurses, therapists, PSWs and other health care professionals undertake every day to ensure more and more Ontarians are getting the best care possible when and where they want it most—in their homes and communities.
